Our first implementation—Community Resource Network in Grand Junction, CO— is a community information exchange that combines clinical, evidence-based precision with the human touch of community-based casework, so we can fulfill the social determinants of health promise: that a well-coordinated, whole-person approach improves overall health and equity across the entire community.

What is a Community Information Exchange (CIE)?

A platform that integrates data from multiple sources (health, social, behavioral) to show a  whole-person view of health. Our CIE makes instant, closed-loop electronic referrals, provides a place for the whole care team to communicate, and encourages proactive interventions for individuals as a solution to community health.

So what? Where is the value?

The value lies in our ability to best serve community members through understanding the whole picture of their needs in health, behavioral health, and social areas.  The more service providers understand about them through sharing data, and coordinating services, the better you can align care. Ultimately the big picture value is improved well-being and social outcomes for some of the community’s most vulnerable individuals. Client’s can benefit immensely from allowing their data to be securely shared among their providers. 

What can I expect to see inside?

Professionals providing care can:

 

  • See a comprehensive view of their patient’s or client’s needs through the One View
  • Search for available resources in your community, send electronic referrals, and see the status of the referral
  • Communicate directly with other professionals working with the client
  • Upload documents for the client in the Document Vault and access documents that other organizations have uploaded
  • Update information about the client or see what updates other organizations have made
  • Enter standard assessment results and see what assessments have already been completed elsewhere
